What Causes Extreme Variation In Temperature?

Dear Dr. Topiwala, I am hoping you can help me figure out what is not well with my body. Thank you for taking the time to help. The following are the symptoms that are greatly affecting my ability to function on a daily basis and are of my most concern: •     My body struggles to regulate its temperature. o     I overheat numerous times per day. It is not like a hot flash. My body will overheat and sweat. This is triggered usually by two different types of things (1) a change in external temperature. For example, I remain extremely hot for 10-15 minutes following a shower and must sit for that time or else my body will sweat profusely. Other times this may happen include coming inside on a cold day. My body will become very hot and sweaty and then will eventually cool off. o     Or (2) an exertion of energy produced by activity that would not normally produce overheating/sweating. i.e. I walk from my car to my desk in my office. By the time I get there I am really, really hot and sweaty. I will then take my body 15-20 minutes to cool off. I have overheated just from packing and collecting my work bags in the AM. I have overheated when doing dishes, vacuuming or simply getting dressed. o     I used to be cold most of the time. For example, my typical dress in the winter would be a turtleneck and wool sweater. Now I can barely wear a cotton sweater. My hands and feet used to always be cold…sometimes cold to the touch of others. Now I often don’t even wear gloves or a hat outside in the winter. It is also important to note that this issue began before I gained any weight. •     Fatigue. o     I have a difficult standing for longer than a few minutes. I become very tired and need to lean on something or sit down – the latter being the preferred. o     I often don’t feel like I am functioning at 100% no matter how much sleep I get. I feel like I push myself all day (for a normal, typical day) and then when I get home I crash. In addition, every so often my body shuts down. Every so often I’ll spend an entire weekend day sleeping out of pure exhaustion. It is also important to note that this issue also began before I gained any weight. These symptoms are additional and I am not sure if they are related or not: •     Groggy thinking •     I have gained about 75 pounds in 3 years with no significant change in eating habits. In addition, I have never been able to lose more than 6 lbs through healthy eating and rigorous exercise. •     I have been sinus infections several times a year in the past few years. Until a few years ago I never recall having had a sinus infection and would not get sick often. I have had my thyroid testing and the results have been normal. I also have been tested for Cushing’s disease and that was negative. Here is the interesting part though – The night before being tested for Cushing’s I took the drug Dexamethasone 1mg as required for the test. The next day I did not have any overheating symptoms until about 5pm. Based on my activities that day, I typically would have had 5 episodes minimum. In fact, I remained cold most of the next day and kept my winter coat on indoors. Something I would never, ever do because I would just be a pile of sweat. Explanation: Your symptoms are suggestive of Gen anxiety/related to thyroid. So, you better go for thyroid function tests another time and please consult psychiatrist if all your tests are normal.
