What Causes Dizziness While Bending?

my husband works outdoors on natural gas pipelines, he s been doing this for many years. Recently he s been experiencing dizzy spells, especially when he bends over. he feels like he may pass out if he doesn t sit down. A few months ago, he ended up in the ER with severe dehydration. His local Dr. re ran labs days later and said ever thing checked out. He ruptured his ear in his teens, could this be an inner ear problem ?
