Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.
Acne, truncal obesity, buffalo humps, high DHEAS and increased
adrenocorticotropic hormone (ACTH) are suggestive of central
hypercortisolism – mainly Cushing's disease. It is overproduction of adrenal hormones in the body. Adrenal gland hormone (cortisole) level in the body is mainly controlled by ACTH from
pituitary gland. High ACTH directs adrenal glands to produce more cortisol which causes rounded face, acne, increased fat around the neck, truncal obesity, fatigue, weakness,
high blood pressure, etc. Besides pituitary gland ACTH may also be produced by other pathologic tissues in the body.
You should have a test called high dose
dexamethasone suppression test. If after the administration of high dose dexamethasone ACTH production suppresses it shows pituitary problem (mainly hormone producing from pituitary adenoma - Cushing's disease). If high dose dexamethasone cannot suppress ACTH production it means ACTH production from a tissue other than pituitary gland (very rare).
Please, consult with your doctor about Cushing's disease and high dose dexamethasone suppression test.
Hope I have answered your query. Let me know if I can assist you further.
