General & Family Physician 's  Response
Hi,

I have gone through your medical history carefully and understood your concerns.

Having difficulty in breathing with a rapid heart beat is indicative for possible:
- arrhythmia (tachycardia)
- associated with angina (or ischemic heart diseases).

Further investigations are needed to determine the right cause including :
- ECG
- cardiac echo
- stress test
- Holter monitoring

You should schedule an immediate appointment with you local cardiologist. If you have any other episode, should run to ER for further evaluations.

Meanwhile, I advise to:
- avoid foods/drinks high in caffeine
- avoid moderate to intense physical activity

Hope it helped!
