What Causes Depression After A Tonsillectomy?

My son had a tonsillectomy 3 weeks ago. He seems to have recovered from the pain at this point, but he is not himself. He is weepy and seemingly depressed at time. He expressed overwhelmed, hopeless feelings and says he feels like he is the only one who knows what that feels like. I am attending to it in a nurturing way without rewarding the negative thinking with too much special attention (so as not to increase or continue it for secondary gain). Is this normal? Is there something about surgery, tonsillectomy specifically, or the pain meds he took for a week that might leave him emotionally depressed? It is truly out of character for him.
