General & Family Physician 's  Response
Hi & Welcome.
It could be due to an early pregnancy or stress.
Withdrawal method is not 100% effective in preventing unwanted pregnancy even it happens with a single encounter. This is due to fact that the pre-coital secretions do contain sufficient number of active and motile sperms capable of fertilizing the ovum (egg) and hence pregnancy can occur. Other causes could be due to stress, infection, or hormonal imbalance. Get her pregnancy test done and consult a gynecologist for further evaluation.
Take care.
