What Causes Confusion, Memory Loss And Fatigue?

My wife is a 46 female who has been suffering from confusion, loss of coordination, mild memory loss, fatigue, loss of empathy and sometimes sympathy. She has been Dx. previously with Vitamin D deficiency, Anxiety, Depression and Adrenal Insufficiency. Her sibling (sister) was been Dx. with Semantic Dementia and ALS at age 49 but looking retro we could see her symptoms as early as 45. Should I be alarmed.
