Since you are having persistent
hematuria (blood in urine) after a negative
cystoscopy and CT scan, it is important to rule out the following conditions:
1. Small tumors inside kidney which were not picked up by CT scan. Did the CT scan include a urography part? This includes a delayed scan after injecting contrast to see the internal structure of kidneys.
Urine cytology also helps in assessing tumors. A flexible ureteroscopy (to look inside kidneys and ureter) is mandatory in case of persistent hematuria.
2. Non-surgical causes - these include kidney problems and blood disorders. Urine microscopy may either reveal dysmorphic (abnormal shaped) RBCs or may not show any RBC (in case of
hemoglobin leak).
3.
Papillary necrosis - more common in diabetics. This can be diagnosed by a flexible ureteroscopy.
