What Causes Cloudy Bloody Urine And LLQ Back Pain?

I had cloudy bloody urine no neutrophils and no nitrates - was treated with antibiotics X 5 days and urine cleared - continued to have occasional visual blood in urine and weekly dip sticks revealed moderate to large blood

cystoscopy was negative and ct scan was negative. Urologist said no casts seen in urine

what is the problem?

I also have LLQ back pain that seems muscular and sometimes radiates to thigh

Hi,
Since you are having persistent hematuria (blood in urine) after a negative cystoscopy and CT scan, it is important to rule out the following conditions:
1. Small tumors inside kidney which were not picked up by CT scan. Did the CT scan include a urography part? This includes a delayed scan after injecting contrast to see the internal structure of kidneys. Urine cytology also helps in assessing tumors. A flexible ureteroscopy (to look inside kidneys and ureter) is mandatory in case of persistent hematuria.
2. Non-surgical causes - these include kidney problems and blood disorders. Urine microscopy may either reveal dysmorphic (abnormal shaped) RBCs or may not show any RBC (in case of hemoglobin leak).
3. Papillary necrosis - more common in diabetics. This can be diagnosed by a flexible ureteroscopy.
