What Causes Chronic Mid And Lower Back Pain?

I have chronic mid & lower back pain, pelvic pain, frequent headaches, excessive saliva & chronic fatigue. Had pelvic & vaginal ultrasound which showed nothing out of the ordinary. Have taken zanaflex, but pain continues unabated. Any ideas what could be causing this? Lower back pain started in Oct 2014. Pelvic pain & headaches started about a month ago. Mid back pain which makes taking deep breaths difficult started two days ago, same with excessive saliva.
