What Causes Chills With No Fever?

yes my brother had a heart attack oct 13th of this year and is with me now on vacation he had 3 stints put in and it was determined he had 30% damage to his heart. He is and has had chilling can t seem to get warm for off and on the last two days but doesn t seem to have a fever? What could it be and what should we do?
