Hi.
Thanks for your query.
Read and understood your history and concern.
The first concern looks to be STD. You can ask your Doctor to advise you on prescription to undergo STD panel tests and the reports will confirm or rule out the STD. This foretells the necessity in you to use condom every time you have sex.

Since you are a known case of prostatitis, it is required to have related tests like:
Tests of the blood for- Routine CBC, sugar, STD as discussed,
Urine : Culture, sensitivity, microscopy and routine.
Ultrasonography of the whole abdomen particularly for Urogenital system, Per-rectal US for prostate and seminal vesicles.
Color doppler and ultrasonography of the scrotum.

I would advise further :
To get the sample of the prostatic massage - this works as a treatment and get a sample of prostatic fluid for examination and culture.
-Long term treatment with suitable antibiotics, plenty of fluids help in good recovery.
I find this answer helpful
