What Causes Bulbourethral Gland Pain With Urinary Frequency?

I have a dull pain in the area of my bulboureathral and it sometimes causes the feeling of needing to urinate. It gets worse after any sexual activity. I have seen a uriologist and have to go back for explorative surgery. What problems could I have and could physical activity make it worse and is there anyway of making this better?
