What Causes Blood In Urine And Swollen Testicle While Treating Quadriplegia?

i am a care provider for a quadriplegia man 58 years old he has used catheters for over 20 years. he has had blood in his urine which is not uncommon but one of his testicles is severely swollen i dont have a way to measure his temp but he if he has fever it is low grade he has reg bowel movement his head is hurting and he is abnormally weak any idea as to what the cause is?

This could be an indirect hernia, a testicular torsion (twisting), or epididymitis--an infection around the testicle,

His doctor needs to know about this right away and he needs an exam and a scrotal ultrasound to determine what's going on here
