1.The possible cause could be superficial or subcutaneous infective aetiology caused bleed, probably hair
follicle infection when ruptures caused bleed.
2.But so long bleed is bit suspicious if there is any thing internaly related for the above cause.
3.In this case
ultrasound of the
scrotum will be helpful and investigation of choice.
4.Ultrasound scan can assess weather its in subcutaneous plane or internal relation is there, testicular parenchyma status, chord, vessels,
epididymis status,any fluid collection can be assessed.
5.The pain could be due to the bleed caused, localised inflammatory changes.
6.Suggest further evaluation and treated accordingly. Even an examination with medical professional will answer most of the questions. Good luck.
