What Causes Bad Smell For The Breath With Uncomfortable Feeling?

I feel like whatever I eat whether it be bland and simple(crackers,bread and water) or fruity I have the smell of that food whatever it was rotting or fermenting coming back up through my breath... Depending on what it is sometimes smells like ammonia sometimes alchohol sometimes DEATH itself! I dont drink alchohol, I dont do drugs, I do smoke cigs. My diet has just recently kicked all caffeine,Milk(which i used to drink tons of)most red meats,and most meats in general. I eat mostly bland foods and drink water. Brush my teeth avg of 5 times a day. I feel as if the smell is by-passing a clean usually gum filled mouth and coming from lower. No pain just an uncomfortable feeling esp. when lying on my back... Please tell me what is a possible solution for WHATEVER I have.I AM WHITE 32 YEAR OLD MALE ABOUT 150LBS(always been skinny) 6ft tall and always walking and burning calories
