What Causes An Itchy Red Rash Under The Armpit Spreading To Collar Bone?

I have had this rash for about 2 weeks now, it is red and itchy and burns when I itch (obviously) but it won't go away... under the armpits and spreading to collar bone/shoulders. I've taken oatmeal baths, used different creams, even vitamin a/d cream.
