Lyme disease usually begin with
erythema migrans ( EM stage I , localized infection )before disseminating ( stage II ) or causing persistent infection ( stage III ) .
Stage I
=The classic presentation is RED MACULE that expand slowly to form an annular lesion with a bright red outer border central clearing.
Central erythema, induration , necrosis vesicular changes or many red rings within an outer ring is also possible.

So if your friend has these symptoms , may have stage I LYME disease .
Stage II is a disseminated infection
=Nonspecific symptoms include
severe headache , mild neck stiffness, fever ,chills , migratory
musculoskeletal pain, arthralgia, malaise and fatigue.
Neurological deficit include meningitis ( ~15% ) ,
encephalitis etc.
