Hi,
Welcome to HCM.

Protected intercourse and a slip of condom has a very low risk for STD/HIV. Even an unprotected vaginal exposure has a risk of 5 / 10000 exposures for HIV as per CDC, Atlanta, USA. Then wearing a condom and had a slip reduce the risk still lower. So there is nothing much to worry.

Regarding her test reliability, it depends upon when she had her last test and when she had her last contact. If she had sex within 10 weeks after the test, there is every chance for her to remain negative in HIV Window period and likely to infectious.
Getting tested and rule out the least possibility is the ideal thing for you to follow.
