ENT Specialist 's  Response
Dear, It seems that you have been suffering from chronic fungal infection in the ear for which you have been taking medicine but without precautions. The hole in your drum are not because of the medicine given by the doctor but its because of the fungus. Noise in the ear could be due to the nerve weakness or simply because of the small holes in the ear.
I suggest taking antifungal ear drops and visiting ENT specialist regularly to remove fungal debris from ear and most important to ensure that no water goes into ear. Keep using the drops even though there is no visible fungus for another 2 weeks. Hopefully this will solve your issues and for noise in the ear, if holes are small they will close and noise will also go away.
