Thin Due To Fast Metabolism, Constant Pressure In Chest, Shortness Of Breath. Used Cocaine Earlier. Thoughts ?

Hello Ive been having a few problems with my body in the last 2 to 3 years that really worry me and make it hard for me to go through day to day life without having frequent concerns.. Let me start off by saying im a 26 years old father of 2 I am not very active im 6ft and 155 pounds ive never stuck to a diet aka ate pretty unhealthy all my life but still thin due to a fast metabolism I pretty much have never taken care or worried about my body and have treated it very poorly through my life so far. I have a history of drug use main drug of choice cocaine I say this because I feel it may be relevant but haven't used since I was 21 and had a scare where I felt I was going to OD after doing to much which is when I chose to kick the habit due to being a new father. (wanted to be around) . I have been a pack a day smoker since the age of 16 although I have taken a month here month there breaks battling and trying to quit throughout my life still no luck but continue taking shots. ok well that is my brief history I hope it helps. ok so I first discovered one of my main symptoms at the age of 21 and it started recently after my drug scare. I feel like my heart beat has never been the same since. (the symptom) My heart rate ever since then has seemed to climb dramatically in pace and seems to palpitate a lot often. I mostly feel the palpitation at night at rest, but feel my racing heart beat throughout the day almost everyday. When I excersize however it seems to climb to an almost scarey rate where if I life up my shirt I can see it though my chest I often get extremely light headed and often panic when I notice this because I can feel my heart rate throughout my body and its to a pace that I am almost certain isn't healthy. This is the reason I haven't excersized in the last 5 years and its really due to fear. I also get tightness in my chest during exersize which is very scarey also. which brings me to my second symptom. I have this constant pressure in my chest throughout the day almost every single day and its very very uncomfortable its like chest tightness all day every day that never goes away. I also at rest have shortness of breath often and sometimes it feels like I am breathing but getting no oxygen which often subsides when I get up and walk around for a bit. I have gone to the ER once or twice just because i dont have a primary doctor and have gotten an EKG? I think that's what its called.. the scan of your heart? but It didn't show anything abnormal which is confusing because I never use to have these problems when I was younger and I never use to feel my heartbeat that hard when I excersize and get tightness in my chest when I excersize. please help. I want to be able to play with my kids again im too young for these kind of things to be happening to me and ive been living with this for the last 5 years. nobody seems to be able to help me.. they all tell me im to young and thin to be having heart problems and that im fine but I can feel something is wrong..

Greetings of the day
As You said you are a chronic smoker, which may be cause of your symptoms- Nicotine present in Cigarettes increases heart beat. EKG taken at rest does not rule out Ischemic Heart disease. Kindly ask for two test from your Physician
1. Stress EKG(Done while exercising)
2. Echocardiography
If the above two test are normal then there is no reason for further concern
