Suggest Treatment Fro Severe Pain During Intercourse

I am a 30 year old female with 4 children . 1 week ago I experienced severe deep pain during intercourse with my partner of 6 years, afterwards I had a sore/ache abdomen lasting days. Since then I have been experiencing night sweats, I can eat more than a few mouthfuls before experiencing early satiety, my abdomen is severely distended and very tender to the touch, I have urinary urgency and my bowel habits have changed from being every morning to 3-8 times a day alternating between mild constipation and violent loose stools. I am taking gabapentin and mebeverine for hypersensitivity of the bowel. Brief History: I had a laparoscopy and endoscopy 10 months ago due to possible inflammation or adhesions of the bowel (12 weeks of diarrhea) (interssusception as an infant and division of adhesions at 12) I suffered a large pelvic haematoma 4 years ago (bicycle straddle accident) a traumatic miscarriage 2 years ago due to retained products of conception. I have had 4 natural births, 3 at home with no pain relief so my pain threshold is quite high. When I told my Dr my symptoms he told me that pain during sex is nothing to do with ovaries, lucocytes in my urine doesn t mean I have an infection? My abdomen is so distended I look 5 months pregnant and it s extremely sore - to the point of not being able to sit, stand or touch it. Dr gave me cocodamol and sent me away. 3 days and my abdomen hasn t gone down. I m worried but the 2 Dr s I ve seen have dismissed my concerns (possibly thinking I m a hypochondriac due to complex medical history) Am I really supposed to just endure this pain and discomfort? ? What could be the possible causes? I do not want to leave these symptoms to worsen. If there s any possibility it could be a UTI or PID I know treating it early is important. I feel at a loss and would like to know if my symptoms warrant a more urgent approach? Thank you in advance for your help.

Hello,

As per my clinical experience, the severe pain during intercourse do indicate vaginal spasm from possible inflammation, scarring or other issues. There can be deficiency of estrogen, age related changes, uncontrolled diabetes, candida infection or others.

After primary physical examination, I do recommend to undergo:
- Complete hemogram
- Urine microscopy and culture
- Vaginal and cervical swab for culture
- Ultrasound of the abdomen and pelvis
