My name is sridevi I am 38 years old I have grade-1 anterolisthesis of l5over s1 vertebra withdefect inbilateral pars interarticularis disc bulge seen at this level causing ventral thecal sac indentation with lateral recess and moderate bilateral neural foraminal narrowing

1. going by your case history, you are suffering from disc prolapse with stenosis producing nerve compression and sooner or later surgery will be required [ if symptoms appears and become unbearable/worsen], till then if you can lead a normal life with the help of medicines ask your attending Orthopaedician/Neurosurgeon about the same, since his assessment is important regarding the need for the surgery.

PS. medicines will help improve the symptoms but will not reverse the pathology.

2. As there is the fusion of L5 & S1 which is causing all the symptoms thus surgery is the choice.
