Suggest Treatment For Vaginal Itching And Inflammation Post Intercourse

Hi.....last Wednesday, had sex with my boyfriend. I always urinate and douche right after as I am prone to uti s, however, it was very late and we both fell sleep for almost 2 hours fterwards. I went to the washroom and douched when I woke up. I also have been battling with vulgar eczema for about 9 months. On Thursday I was ok. Late Friday, I had some some vaginal itching, which proceeded to get worse and woke me up in the middle of the night. The itching was the worst just inside the vaginal opening and a centimetre or two inside. By Saturday afternoon, it was very itchy an very sore, so I picked up a 3 day combi pack of Monistat as it was very symptomatic f a yeast infection. I ve nly nice before had t treat a yeast infection and that was back in 2004. By sat night and Sunday morning, things were intensely painful and itchy. I ve been treating with both Monistat ovules and canes fan vaginal cream. The vaginal cream has seemed to help the most. I ve also had some bleeding/oozing, especially just inside the vaginal walls, but that now seems to be under control. My labia, the thick area just inside the vagina and my perineum or incredibly infammed and so sore I can barely walk or sit. Because of the the anti fungal creams and treatments, I believe this exacerbated the eczema and caused the secondary inflammatory and extreme pain and burning. My perineum is so swollen that it looks as if my labia thinly continues all the way to my anus...not good. Currently, I am using both the anti fungal, plus a water based, No perfume , dyes gel vaginal moisturizer plus my eczema cream (mometazone) to bring the inflammation down plus a cool cloth for relief. I think/pray the worst is over. Have you heard of this before and/or do you have any suggestions? Any help would be greatly appreciated. Thank you so much.

General Surgeon 's  Response
Hi,

This is usually self limiting and may not indicate any disorder however, in some cases urinary or std may be present if this last for more days.

I will suggest you keep the private parts clean and dry. Use of Lactic acid containing vaginal wash at Bath time is advisable. Use of Condom, over the period of the next 2 weeks while having sexual activity is advisable.

I STD tests are done then in case of positivity you can have a week long course of Doxycycline 100 mg. orally twice a for a week. Antifungal agents like Fluconazole 150 mg once a week.

A local ointment containing clotrimazole and clindamycin twice daily for a week to be applied on the private parts and vaginal tablets of Clotrimazole and Clindamycin at night for 7days.

Please seek prescription help from your treating gynecologist for getting oral, local and vaginal antibacterial and antifungal agents.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Ivan R. Rommstein, General Surgeon
