Suggest Treatment For Uncontrolled Diabetes While On Insulin

Hi. I am diabetic since last 25 years. I am on insulin for last 10 years. I am taking Human Mixtard 70-30, Morning- 45, Evening- 40. along with Azulax-2 mf forte twice daily. I am taking very limited diet. But my sugar level is not controling. It is around 220 Fasting and 410 pp.

Dear Sir/Madam,
Welcome to HCM,
I understand your concern. Sugar control is of atmost importance for prevention of Diabetes related complications. Few factors you need to take into consideration before altering dose or regimen is
1. Whether the sugar control became back after starting new insulin bottle /pen. If so please check the expiry date of insulin and also if there are any clumps in the bottle or pen even after rotating in palm of hand. in this case you need to start with a fresh bottle/pen and you will be fine.
2.Whether the insulin leaks out after prickinging. This means the adequate dose is not entering your body.
3.Are there any lumps or fatty swellings at the site of insulin injection?
If the answer for all the three above statements is negative then you need to change the regimen. Our body needs more amount of insulin due to lack of insulin from the pancreas so the best regimen is called the Basal -Bolus regimen. This means you take three doses of short acting insulin before food and one dose of long acting insulin at bed time.
This dose can be brought down once sugars are under control and if good control is obtained you can even go back to Mixtard 30/70 twice daily.
I understand that i have given you an option with more doses of insulin. But since your sugar levels are pretty high and considering the duration of Diabetes this would be more appropriate regimen.
