As per your signs and symptoms it looks like the fungal or bacterial infection. If it is white lesions on the tongue it may be the fungal infection because antibiotics some time lead to opertunistic infection of candiases on the tongue. Which may or may not be painful. The swollen lymph nodes can be also because of it. Yes, You need to visit an general physician because the swollen glands are not good signs, mostly if they are painless though in your case it seems to because of infection, sore once.
