Suggest Treatment For Swelling In The Joints And Rashes On The Feet

My nephew is in China. He was taken to the hospital with these symptoms: swollen joints, aches with a feeling that he couldn t move, fever of 102, rash including on the bottom of his feet. He has a history of asthma and was so sick this past fall as a freshman in college, that he passed out and was taken to the ER with a bad cold and high fever. This most recent episode in China came after three days of not feeling himself. Apparently, they have given him fluids but he is not urinating so doctors in China are going to do a pelvic ultrasound. An infectious disease doctor will be examining him. Any clue as to what he may have? Could it be Lyme Disease? Could it be a re-emergence of the bad cold that he had in the September where many fellow classmates also fell ill and the dorm he was in had moldy air conditioners? Any ideas?

General & Family Physician 's  Response
Hello and welcome to HCM.
Sorry to know about your nephew.
The clinical scenario you described looks like he must be having viral haemorrhagic fever like dengue, chikungunya transmitred by mosquito. In lyme disease, there is history of tick bite and rash is limited to the bite site in most of the cases.Though we cannot completely rule it out.
Basic and specific blood tests will rule out the disease.
In chikungunya, illness is generally limited to 7-10 days.
The prognosis of dengue depend upon the clinical and blood parametres of patient.May require vigourous in patient follow up in hospital with iv fluids, platelet transfusion in case of haemorrhagic complications(renal failure as one of complication causing decreased urination).
If detected with lyme disease, course of antibiotics will generally help.
Hope this information helps.
Wish him speedy recovery.
Thanks
