My father is aptient of COPD. The brif history is mentioned below :

PATIENT NAME      :      AMIYA KUMAR MUKHERJEE

AGE               :     74 YEARS

SEX               :     MALE

HABBITS          :     SMOKING

PRESENT STATUS     :     LEFT SMOKING 1 YEAR BACK





PATIENT HISTORY :



2009 :



HE WAS HAVING SEVERE PAIN ABOVE HIS LEFT EYES. HE WAS SHOWN TO SPECIALIST ON NEURO MEDICINE NAMELY PROF. SHYMAL DAS. TILL DATE HE IS UNDER HIS TREATMENT FOR THIS.

2010 :



HE WAS ALSO DEVELOPED ACUTE BREATHING TROUBLE. HE WAS SHOWN TO A CHEST SPECIALIST NAMELY DR PARTHASARATHI BHATTACHARYA. TILL DATE HE IS UNDER HIS TREATMENT FOR THIS.

2011 :



SUDEENLY DURING LAST WEEK OF JUNE HE WAS DIAGONSED WITH HIGH PRESSURE ( 200 / 140 ) WITH BRONCHIAL DISORDER. LOCAL DOCTOR WAS SHOWN. SUBSEQUENT TO INTAKE OF HIS MEDICINE PRESSURE FELL VERY SHARPLY (96 / 60) AND HE WAS VERY WEEK.



IMMEDIATELY ON 29.06.2011 HE WAS ADMITTED TO FORTIS HOSPITAL UNDER DR YASESH PALLIWAL.



ALL PRESCRIPTIONS / TEST PERFORMED SINCE BEGINNING DURING HIS STAY IN FORTIS HOSPITAL IS ENCLOSED UNDER ANNEX –C ( ZIP FILE – FORTIS.ZIP ).



HE STAYED THERE FOR 8 DAYS AND AS HIS CONDITION WAS STABLE HE WAS RELEASED ON 06.07.2011.



PRESENT CONDITION :



CONDITION           :      STABLE

PRESSURE           :     130 / 60 ( 09.07.2011 )

SUGAR          :     128 ( TAKEN AT 9.45 P.M BEFORE DINNER ON 08.07.2011)



OBSERVATION     :     HE IS IN CONSCIOUS STATE OF MIND. ABLE TO

                    WALK, TALK, INDENTIFY THINGS. ONLY HE IS ALWAYS

                    FLEEING VERY SLEEPY AND SLEEPING ALL THE DAY.

as your father is a smoker and diagnosed to have copd, the reasons for increased sleepiness will be
1. carbon dioxide retention - as he is stable now get a ABG(arterial blood gas) analysis done in any hosp.. if co2 is high (more than 45) his copd treatment is not optimum.
2. overlap syndrome - copd with obstructive sleep apnea. it is common to have sleep apnea in pts with copd. if he has excessive day time sleepiness, choking, snoring, fatigue, tiredness or comorbidities ( diabetes, htn, stroke) he shud undergo sleep study which will diagnose the case.
