Suggest Treatment For Severe Rectal Pain

I have severe rectal pain and have been diagnosed with a tight rectal sphincter.Physical therapy consisting mainly of rectal stretching has not really helped. I take Miralax with frequent bowel movements often with hard button like stools. I have had closure of my vagina for a prolapsed cervix after hysterectomy including a sling for bladder support. I have seen a number of gastroenterologists and colorectal surgeons to no avail. Can you help?

Have your doctors considered a nitrate ointment or botox injections? Alternatively, a sphincterotomy could be considered, but you need thorough consultation; seek third (or fourth) opinion if no one helps.
