Charcot joint also called Charcot neuropathic osteoarthropathy. Charcot joint is classified into two types, neuro traumatic and neuro vascular. In neuro traumatic, repeated trauma to the foot and ankle due to neuropathy of the joint results in dislocation and
fracture of the joint which ends in
deformity. In neuro vascular type, there is increased blood flow to the joint which increases the bone osteoclastic activity that is destruction of the bone because of demineralization.
Diabetes, neuropathy, trauma and metabolic abnormalities are the causes for Charcot joint. Due to the absence of necessary hormones and its related action such as
calcitonin gene related peptide which inhibits the
cytokine causing inflammation of the joint. Absence of CGRP results in high inflammatory cytokine affecting the joint which in turn causes dislocation and fracture of the joint. CGRP also maintains the integrity of the joint capsule. Imaging studies and metabolic studies helps to confirm the condition and treat the defect earlier to prevent any deformity.
