There are various ranges of blood pressure alteration which can be categorized as mild, moderate, severe, malignant hypertension. As there is narration that person is not able to know where he is and where is he going, altered mental status is most likely in range of severe or malignant hypertension with or without underlying vital organ damage. Treatment is best reaching an ER for prompt control of blood pressure via sublingual medications and avoid further damage to brain and other vital organs of the body as heart, kidney and others.
