Suggest Treatment For Schizophrenia

Dear Doctor,

My father aged 68 was diagnosed with a psychotic disorder most probably schizoprenia in this late 30s. He was prescribed espazine plus, which he took on and off till date. However last year he had a Cabg surgery and was on a lot of other medications and he skipped espazine plus. Its now more than a hear.

We see now he has become very violent, hallucinating all the time, very agressive with family members. He is doubting every body and started cursing and threatening others.

Should we start him back on espazine plus or we need to get him reevaluated. There are changes he wont go to the doctor with us now.

We are very worried. Pls advice

Hello Abdul,
I think there is resurgence of symptoms of schizophrenia in your father. Such is the nature of this illness that the symptoms reappear when you stop taking antipsychotics.
Since your father has had CABG in recent past, i would advice you not to start with espazine again as first generation have a higher risk of causing heart problems. Better option would be second generation antispychotic like risperidone given in doses of 2mg-4mg. Here i would also advice you to go for 3monthly checkup which includes blood sugar, ecg, lipid profile and electrolytes.
