Suggest Treatment For Recurrent Paronychia On The Large Toe

I m having reoccurring paronychia to large toe. To point of pushing nail off. I went away a few weeks ago and now is back again. I went to walk in today for antibiotics before getting bad like last time however it is now again pushing my nail away and the nail is cutting into it. Should I go to ER? I have no insurance. But I m an RN...what can I do, soak, elevate, keep on Bactrim?
