Suggest Treatment For Persistent Nightmares While On Lexapro

About 7 months ago My 13 year old granddaughter who has always been an outgoing bubbly child became afraid to go in the bathroom alone , complained that she was afraid to be alone, has been having nightmares, and then started talking about suicide. He dad took her to the emergency room and she was admitted to a mental facility for evaluation. She stayed there for 6 days. They started her on 5 mgs of lexapro. She was released 3 days ago. This evening she got angry for no apparent reason. Se lives here with me, her dad, older brother and older sister. She is fixated on me. She kept saying that I don t love her that I don t care about her and the more we tell her that isn t so the more she believes it is. Then she started saying several times that she wanted to die, that she would e better off dead. That she is going to run away and walk out in the road and let a car run over her. Now we are watching her closely because she has an appointment with her therapist tomorrow at 3:00. I feel like she is getting worse than she was before she went to the hospital. I know that the medicine package says not to discontinued the medicine abruptly. My son has been giving her a dose in the morning around 6:30 am. My question is would it be alright for her to skip it until she sees the doctor at 3:00 pm and let the doctor decide what to do? In other words would it physically hurt her to miss the dose? Never mind.....I think I will try one of the suicide hotlines

General Surgeon 's  Response
Hi,

Yes, these could be the side effects of the medication. It would be particularly severe if you were taking any kind of antidepressants for the first time. In my patients usually, these side effects subside in just about a month and if not I would reduce the dose to 5 mg a day and then go up to the therapeutic dose even more slowly than I would with patients who tolerate it more.

If the symptoms still persist, your doctor should change it to some other antidepressants, there are many different mechanisms of actions.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein,
General Surgeon
