Hi,

Without complete physical examination, presently I can tell you that it might be a gastrointestinal disease. The following is my opinion:
- Ask for ultrasonography of HBR (hepatobiliary region) for amoebic liver abscess, cholecystitis (as pain is going back) and associated appendix problem
- Pain could be a pleural pathology; ask for a chest X-ray to exclude pleuritis, pleural effusion
- If the investigations come normal, take an opinion of a gastroenterologist or medicine specialist who might ask for an investigation like endoscopy, gastric analysis colonoscopy

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
