Suggest Treatment For Nausea And Vomiting Post A Gallbladder Removal Surgery

I had my gallbladder removed a year ago and was also told I had pancreatic injury at that time. I ve still continued to have pain in my left side, radiating to my back periodically. Two days ago I awoke with intense pain in left side, radiating to back again, but also with nausea and vomiting. I ran a temp. Of 100.3-100.6 until this afternoon. I have been unable to eat due to the nausea, and then today began having loose bowel movements. Should I be concerned that this could be related to my pancreas? Thanks in advance.

Thanks for your query.
Noted the history of gall bladder removal a year ago and had pancreatic injury, at the time of surgery, there is pain on the left side, radiating to the back, with nausea and vomiting, then fever of 100.3 to 100.6, unable to eat due to nausea, started loose bowel movement in the morning, wants to know whether to be concerned and related to pancreas...
The pain on the left side that radiates to the back may be due to pancreatitis or gastric ulcer on the posterior wall of stomach.
This can be diagnosed by the following:
- X-ray of the abdomen in standing position to rule out any intestinal obstruction.
- MRCP by MRI to delineate the pancreas, liver and so on.
- Contrast enhanced CT scan.
Tests of blood, amylase, lipase, liver and kidney functions.
- Upper GI Endoscopy to rule out any gastric ulcer, particularly on the posterior wall which mimics the symptoms that you have.
Get a course of a PPI like Omeprazole.
Domperidone at 8 hourly interval to see if this helps.
If there is obstruction, needs release by surgery.
If there is obstruction to the pancreatic duct needs stenting to relieve the pain and swelling.
Thus the overall treatment will depend upon the exact cause found.
