Suggest Treatment For Hyperinsulinemic Hypoglycemia In An Infant

I started a 4 month old infant with hyperinsulinemic hypoglycemia with known genetic mutation after near total pancreatectomy, on octreotide dose of 1 mcg/kg/dose, BID today. Patient still have significant lows despite the pancreatectomy. How long do I wait to titrate the octreotide dose? What is your recommendation as far as using long acting octreotide?
