Suggest Treatment For Hip Dysplasia In Child

hi , im a mother of 3 years old baby girl who has been daignosted as a hip dysplasia and this is her report Clinical Assessment: On clinical examination, we can see that the spine is straight. In the forward- bending test, there is no thoracic or lumbar hump, no signs of scoliosis. In the upper extremities, there is no axial or rotational deformity. All the joints have regular range of motion, we could see no lumbosacral stigmatization. In the lower leg, we can see a regular axis with an inward-rotated gait with signs of coxa antetorta. Bilaterally, we can see a mild decrease of the medial arch, which is age-related and needs no therapy at the moment. Range of motion of the hip, knee, and ankle joints is physiologic, but we can see an inward-rotated accentuated range of motion of the hip joint with an outward-inward rotation of 70-0- 90 degrees. Investigation: As there has not been a hip screening done before, we did an x-ray of the hip today. Here, we could see bilateral disrupted Menard-Shenton line and an AC angle on the right side of 29 degrees and on the left side of 26 degrees with mild elevation of the lateral borders of the acetabular roof. do you think that she needs surgary

Orthopaedic Surgeon 's  Response
Hello,

Hip dysplasia in children is treated mainly for containing the head of femur with respect to the acetabulum for getting a congruent joint. This allows the development to proceed as normally as possible. The AC angle in average is 28 degrees to start off with but decreases with age.

The rotation of the hip becomes better with time as well. Since there is no X ray attached, it is difficult to comment on the need for any surgical intervention off hand with the just the values given by you, so I would suggest a pediatric orthopedic surgeon. Any surgical intervention is decided based on the clinical status of the patient as well the X-ray.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Raunak, Orthopedic Surgeon
