Suggest Treatment For Heavy Vaginal Bleeding And Uterine Polyps

I have had long (approx. 7-10 days) heavy periods for about 2 years now, gradually getting heaver, so heavy that I have to wear a tampon and a child's nappy at night time. I have constant spotting throughout the month (brown and red). I have been to see my gynae doctor who done an internal and found some polyps on my cervix which she removed there and then, I had a trans vaginal scan which she found nothing out of the ordinary. Since seeing my doctor I am having one period with a 2-3 day break and then start bleeding again (heavy) which starts with blood clots a bit bigger than a 50 pence piece. I am extremely tired and have terrible back ache and period pains throughout the month. Please help as I am not sure what to do or what other tests I should request? I am 36 and weigh 54 kilo's
