Suggest Treatment For Headache With Visual Disturbances

Hi, I have had a severe headache with visual disturbances and facial pain since November of 2013. CT Scan in April when compared to CT in 2011 revealed 3 areas of demyelination and the cerebellar tonsils are lower. Went in for an emergency MRI 5 days later and bloodwork to rule out Vasculitis. No inflammation so no vasculitis. I have a 10 month wait to see a neurologist to have my MRI read and possibly diagnosed. I have a history of atypical migraines and also have hemochromatosis. My mother has MS but I have been told it isn t MS. I am currently on 35 mgs of Nortriptyline a day with mixed success after trying Amytriptyline and having too many side effects. Previous CT was due to a nerve problem in my left arm that has since gone away. I have some cognitive impairment....trouble finding the right word, easily become disoriented, and trouble concentrating and comprehending. Not sure if this is due to the medication or not. I have a few questions.... 1. should I have the expectation that this will continue to get worse? 2. do you have any idea what may be causing this? 3. Do you have any suggestions on how to decrease the pain? Thank you for your help

Hello,

They seem more like a migraine. Tiny white matter hyperintense lesions on MRI can occur in a migraine too. Part of the symptoms is due to the side effects of nortriptyline. Stop it and replace with another drug such as topiramate or divalproex.
