Hello and welcome to HCM,
A previous biopsy report showing intestinal metaplasia and current mucosal irregularities in the bladder mucosa could possibly be a growth in the bladder because intestinal metaplasia in urinary bladder is a risk factor for developing bladder cancer.
Thus, biopsy of the mucosal irregularities is strongly indicated.
Mildly enlarged prostate and poor urine flow are indicative of prostatic hyperplasia, however, a PSA level needs t be done.
Thus, you need to go for biopsy from irregular bladder folds and PSA levels as initial investigations.
Further management can be planned after these investigations.

Thanks and take care
