Suggest Treatment For Feeling Of Tightness And Pain In Gums And Cheeks

I also have gums and teeth's problem now in my tighten side of a mouth it seems like my inner part of my chicks they are damaged by my teeth or what I'm not sure but there's a whole there and something white inside there and it painful can't even eat I lost weight now

I have gone through your query and can understand your concern....

As per your complain it seems that you are suffering from ORAL SUBMUCOUS FIBROSIS along with LEUKOPLAKIA [if there are white patches] which is characterized by formation of thick palpable fibrous bands inside the cheeks which makes the cheeks taut and there difficulty in opening the mouth along with atrophy of the cheek mucosa leading to burning sensation and pain on eating even mild spices..
There can also be white or blanched appearance of mucosa..
It is a precancerous condition and can be seen commonly in cases of tobacco and betel nut chewing while at times long standing malnutrition and excessive intake of spicy food has also been considered a cause but has not yet been completely proven..

The other thing that you pointed is that white thing over cheeks which can be the white or blanched appearing mucosa and can be due to Oral Submucous fibrosis while if there is formation of white patches it can be due to Leukoplakia which occurs due to over production of keratin as a result of chronic irritation over the area..

You should consult an Oral Physician and get evaluated and in case of OSMF you can be advised to apply a steroid based ointment like Kenacort along with HYLURONIDASE containing ointment as it helps in breaking the fibres..
Weekly injections of steroids and hyluronidase can also be given under the mucosa in the area of the fibres in order to break them..
Anti-oxidant like Lycopene has to be taken..
Mouth opening exercises will be demonstrated to you..

In case of leukoplakia along with treatment of OSMF oral intake of retinoids and multivitamins and application of steroid based ointment is advised..

Complete withdrawal from tobacco and alcohol and taking low spicy food is indicated..

Continuous follow ups with the treating doctor is a must to get evaluated for the outcome of treatment as both are pre cancerous conditions and in few cases can turn into oral cancer..
