Suggest Treatment For Episodes Of Randomly Occuring Migraines

My daughter has suffered random migraines for a few years. Recently she experienced one that was so bad that she experienced a state of confusion, didn t know where she was, thought someone took her hands, her personality changed. We took her to the ER and they did a CT scan, everything checked out and she was hydrated. However she had lingering effects the rest of the week and then had another migraine a week later, not as severe. From there she complained of pain in her jaw, next day said her quads felt week, next day feeling a little better slight dizziness. Then she played in her soccer game about 15 min and another migraine came on so we left. She took endurolytes prior to the game and one excedrine, in the past she had done this and it seemed to work. Today it didn t. I am very concerned and feel at a loss on what to do.

Hello!

Welcome and thank you for asking on HCM!

I carefully read your question and would explain that it is necessary performing further tests to investigate for the possible cause ( an infection, inflammation, etc.).

I would recommend performing these tests:

- a brain MRI study (as brain CT scan is a test performed easily in the ER but not the test of choice in migraine)
- further blood lab tests (complete blood count for anemia, PCR, sedimentation rate for inflammation, thyroid hormone levels, blood electrolytes, fasting glucose).

An abdominal ultrasound for her ovaries would help exclude possible hormonal disorders, which are known to trigger migraine.

Meanwhile, I would recommend avoiding all the possible trigger factors for migraine ( sleep deprivation, dehydration, certain foods ) and avoid straining physical activities.

You should discuss with her doctor on these issues and on the possibility of starting a preventive therapy for migraine (betablockers, antidepressants, anticonvulsivants, calcium channel blockers, etc.).

Hope you will find this answer helpful!
Kind regards,
Dr. Aida
