WHEN I SPEAK MOST OF THE WORDS ARE NOT ABLE TO UNDERESTOOD BY THE PERSON WITH WHOM I AM talking that time i feel very ashameed i am facing this for last 5 years day by day its becoming worse now a days my parents are also angry with me i have never consulted with any doctor

This is surprising that you are carrying on since last 5 years like this.
This may be an early indication of an underlying brain pathology.
You must see a neurologist as soon as possible, he might advice you investigations, like Ct scan brain or MRI.
Once it is decided if it is pathology or not then only further course of action will be decided.
It is also caused as side effects of some drugs.
You may need help of a speech therapist.
