Suggest Treatment For Difficulty In Breathing And Bloating During Dialysis For CKD

hi. my mother is a ckd patient in dialysis for the last 8 months, for the past 1 or 2 weeks she has been having difficulty in breathing and a slightly bloated stomach. she was advise to take a lung xray and the findings were as follows: both CP angles are blunted by small pleural effusion, heart is enlarged and aorta unfolded,prominent bilateral perihilar vascular markings are noted radiating into the lung fields associated wit some mid zone lung shadowing.. her hb was 8 some 2 or 3 weeks back.. what does the above mean and whats the treatment? thanks, rukaiya

Hello,

The chest X-ray says your mother has a mild pleural effusion with prominent broncho-vascular markings. This is seen in CKD due to dysfunctional kidneys, water is not excreted and thus it accumulates in lungs, infections of lungs like pneumonia, ARDS, right heart failure, and auto immune diseases like sarcoidosis, SLE.

Here your mother's CKD is the main culprit. As she has been on dialysis, the water must not accumulate. It indicates inadequate dialysis, increase in frequency of dialysis necessary, and any underlying heart disease and lung infection that must be investigated

I would recommend immediate dialysis, ECG, serum creatinine, and 2 D echocardiography. Please do consult a nephrologist first to identify the cause of accumulation of fluid in lungs. She requires detailed clinical examination.
