Suggest Treatment For Cough And Recurrent DVT

hi I am male person 55 years old have recurrent DVT of unknown etiology and also have DM , HTN, and on xarelto since 2months, also complaining of irritating cough since few months and CT chest revealed bilateral hilar and mediastinal LNS largest subcarinal about 4 * 3 cm and I will do Mediastinoscopy for LNS biopsies and I ask when I stop xarelto before endoscopy and when I start after and whate your opinion about the diagnosis and DD thanks alot

Hi,

The biopsy is must to rule out any possibility of lymphoma. Xeralto may be stopped three days before mediastinoscopy and can be restarted after 48 hours post procedure. Rest of the management plan as you shared is satisfactory.
