Suggest Treatment For Coronary Artery Disease

I have CAD and a stent in my LAD and another 50% blockages. I am experiencing sudden, stabbing pains in the middle of my chest. Is this my heart or should I ignore it. The pains are severe. They require two nitros. One is not sufficient. I am almost incapacitated when they happen. I have to nitros quick or they will not quit. I have not been to the dr when they happen, for by the time I have had two, they are over. What would be the right thing to do? I had another one this week.
