Suggest Treatment For Chronic Pain And Manic Depression

My fiancee is a 54 year s old male with known dormant hep c, chronic pain, rods in back, past merca, bipolar, manic depression, and a non reversible stoma and has chronic blockage issues. At this time his stoma is raw and bleeding. Severly swollen feet, knees and hands. A large protrusion on his wrist by thumb. That has gotten bigger within past few hours, his hands, feet abd knee have also gotten bigger. He I d refusing to go to hospital. He is in severe pain. I am concerned what should I do? What could be causing these issues?

The swelling hands, feet with above mentioned features, indicate significant medical disorder. The most common possibility sounds to be related to side effect of certain medications used for control of manic depression.
