Hello and welcome
thanks for posting your question

Looking at his history this could be a case of lung cancer or a lung abscess
his symptoms and history of smoking is very much suggestive of a long cancer

in my opinion you should visit a pulmonologist for the examination and investigations like X ray or CT scan chest as well as sputum culture and microscopy

meanwhile you can advise him to do steam inhalation and a cough syrup like Bromhexine

Hope this addresses your question
wishing you bot a very good health
