Suggest Treatment For Breathlessness, High BP And Low Pulse Rate

*My grandma is about 70 years old and she s having shortness in breath, high blood pressure and low pulse rate and pressure (there s a difference between her right arm and left arm s pulse, one is strong & the other is faint). *Her echo indicated Grade I Mitral Valve Regurge. *Her tests showed elevated Ionized Calcium, Uric Acid, ESR, CRP and positive D-Dimer Test. *But her Creatinine Phosphokinase levels & Thyroid levels are normal (TT3, TT4, TSH, Free T3, Free T4), as well as a normal Complete Blood Picture. I m an Egyptian 2nd Year Medical Student so i would appreciate the help diagnosing her disease, even if you had to use medical terms for clarifying further details. Thanks in Advance.

General Surgeon 's  Response
Hello,

The symptoms of breathlessness, high BP with low pulse rate and difference in between the right and left upper limb indicates possible issue at aorta, root, isthmus or trunk more likely. Aortic calcification at 70 is most common which will serve as Aortic regurgitation and Pulsus Bisferience or Aortic stenosis with pulsus parvus at tardus.

The remote possibility of Takayasu arteritis induced secondary coarctation of Aorta is also possible issue with grandma. She requires intensive evaluation with cardiologist ahead for final diagnosis and cure accordingly.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
