I suspect that it is a malignancy of some kind. But it requires further confirmation by radiologically and biochemically.
I advice you should test for PSA and USG abdomen and pelvis. PSA is a cancer marker prostate malignancy. USG will help diagnosis any other pathology.
It may also be possible that it is some kind of infection. You should diagnose the condition by visiting the nearby doctors.
If your symptoms and sign is suggestive of the STDs, you should take the Doxycycline 100 mg once a day for seven days.
